Delmarva Foundation for Medical Care (DFMC, link: qlarant.com) was Maryland’s federally designated Medicare Quality Improvement Organization (QIO).

Delmarva Foundation worked with communities in Maryland to improve patient outcomes for Medicare and Medicare/Medicaid beneficiaries with serious chronic illness and those diagnosed with Diabetes, chronic Obstructive Pulmonary Disease, Congestive Heart Failure, and Chronic Kidney Disease.
